Erythema Infectiosum

This patient presented with complaint of a red and mildly itchy rash with a lacy appearance on her arms and face that had started that day at school. The patient’s mother also states she had a low grade fever a few days before the rash came up.

Erythema infectiosum ( also called Fifths Disease) is a viral rash most common in school-aged children. The rash usually appears as a red rash on the face that looks like the cheeks have been slapped, and can also spread to the arms, legs, and trunk. The rash generally appears as red blotches and then as it fades turns into the classic “lacy net-like appearance” pictured in this patient’s photos. The rash may or may not be itchy. The virus may begin with cold like symptoms such as fever, cough, or runny nose but many have no associated symptoms. The rash normally appears as the cold symptoms fade.

The course is generally benign and recovery is quick and without complications. The rash can take between 1-3 weeks to fade. A person is most contagious before the rash appears and transmission is through secretions from the nose, mouth, and throat. There is no vaccine and transmission prevention is best through good hygiene and frequent hand washing. No treatment is normally necessary, as no antibiotic will help cure the virus. Supportive measures such as anti-itch medications and Tylenol or Motrin are sometimes needed for fever. Once a person has had the virus and is immune, they are not likely to get erythema infectiosum again.
